Executive Director

Job Summary:

This position will assume overall responsibility and authority for administrative, financial and leadership functions of the Putnam County Homecare & Hospice Agency. Typical hours would be Monday through Friday, although situations may occur which would require off hours attention due to the nature of the health care field. This position reports to Homecare and Hospice Board of Health and works in collaboration with the Medical Director. The preferred qualifications are:

• Prior Home Health Management (5 years)
• Minimum of Bachelor’s degree or higher in Nursing and/or a Master’s degree in related field.
• Licensed Clinician in the state of Ohio with 3-5 years of experience in the home care setting.

Salary commensurate with qualifications and experience.

STNA – Part-Time (Lima Area)

Putnam County HomeCare & Hospice is seeking dependable and reliable nurse aide applicants with a genuine interest in hospice to work part-time hours in the Lima and greater areas. The STNA will work day shift, Monday through Friday, schedule depending.

Job Duties May Include:

• Provides personal care and assistance to patients receiving home health or hospice services (for example, bathing, skin care, nail care, toileting, ambulating, meal planning and preparation)
• Monitors patient condition in accordance with the established patient care plan
• Provides various home maintenance and light housekeeping services as part of the designated patient care plan (for example, vacuuming, dusting, mopping, laundry, cleaning bathrooms and kitchens)
• Plans and records daily activity schedule in accordance with the established care plan
• Assists patients and caregivers in ensuring that the necessary materials and supplies are kept in the residence (for example, grocery shopping, picking up medications)
• Provides supportive care and companionship (for example, listening and reading)
• Maintains required licensure and certification
• Demonstrates regular and predictable attendance

Qualifications:
• Completion of a secondary education (high school or GED)
• Previous homehealth or hospice experience preferred, but not necessary
• State of Ohio Nurse’s Aide (STNA) Preferred (Agency will reimburse STNA for the classes upon completion. Employee must remain employed for 1 year. STNA must be passed before employment can begin.)
• Valid State of Ohio driver’s license and insurance
